Preschool registration for the 2023-2024 school year opens today, Monday, April 3rd!!
Please complete ALL of the following steps for your child to be considered for the preschool program. Completing this application does NOT guarantee placement in our preschool program, but gives us the information we need to approve your application. You will be notified by June 1 of application approval in our preschool program. All forms MUST be completed and documents must be uploaded by Monday, May 15th to be considered for admittance into our preschool program.
New Preschool Applicants:

Set up a Parent Account and Create a New Student Account in the 2023-2024 School Year (see blue square).
Please make sure all forms are complete and shown as “green”.
Be sure to click on the link on the Preschool Application Form and complete it.
Proceed below Returning GEVSD Preschool Applicants.
Returning GEVSD Preschool Applicants:
Complete FinalForms: https://greenfield-oh.finalforms.com/. Our returning preschool students who are staying in preschool during the 2023-2024 school year do NOT need to reapply as new applicants, you only need to complete the forms assigned to you in your current FinalForms account for the 2023-2024 SCHOOL YEAR. If you have trouble logging in to your account please use the ‘forgot email’ or ‘forgot password’ prompt to the right of the login area.
Click on the 2023-2024 school year (blue square).
Please make sure all forms are complete and shown as “green”.
Be sure to click on the link on the Preschool Application Form and complete it.

Please upload all of the following documents to your student’s document folder in FinalForms.
Birth Certificate
Immunization Record
Proof of Residency
Drivers License
Custody Paperwork (if applicable)
Proof of Income (paystub, tax documents, etc.) or Award Letter from Government Programs (i.e. SNAP, PFCC, OWF, etc.)

Attend Mandatory Preschool Registration Event:
Greenfield Elementary: Tuesday, April 18 from 4:00-6:30 PM
Rainsboro Elementary: Thursday, April 20 from 4:00-6:30 PM
